For Scholarship Recipients from 2013-2024 ONLY

In order to make receiving your scholarship as seamless as possible, please follow these instructions.
The award monies may be used for tuition, textbooks (on-line or actual), and/or school authorized
housing and meal programs at Florida institutions of higher learning. We don’t mail any funds directly
to students except as reimbursement for funds spent. In that case, a receipt must be submitted with
your request.

Your scholarship benefits may not be used for repayments of student loans.

1. For each request, please complete a copy of the enclosed Student Request Form. Please
complete it clearly to prevent errors. Be sure the address you list as the school’s payment’s
mailing address is correct.

2. For tuition payments, attach the school’s invoice which must have your name and student
number on it as well as the semester date.

3. Again, if it is a reimbursement, attach the credit card receipt or school’s receipt.
